All client forms placed on your eVetsite should be in .pdf (Adobe Acrobat Portable Document (PDF)) format. In this format clients can easily view and print out the form and CANNOT make any changes to the form.

To convert any form, newsletter or other document to the .pdf format, we recommend a simple free shareware program that you can download in seconds called 'CutePDF Writer' from the CutePDF website, which creates a printer driver tool that you access from your print command:

After you download the .pdf tool, start by creating your document in MS Word, MS Publisher or any other document program.  Click on print like you were going to print the document.

Now choose CutePDF Writer in the pull down menu instead of your regular printer driver and then click OK.  The next screen will ask you to Name the document and navigate to where you want the new .pdf document to be saved to:

When you click on Save the new tool will convert your document to the .pdf format and save it to your hard drive.  Now you are ready to upload this .pdf to your eVetsite for use by visitors to your eVetsite.
